I acknowledge that I have read and understood the privacy statement, and consent to the collection, use and disclosure of my personal data by UOB Art Academy for the purposes set out in this website.

本人同意在收到由大華銀行「藝 • 坊」發出的付款通知書後的兩天內，透過指定方式付款以完成報名程序，並知悉報名一經確認後不得更改至其他工作坊或取消，所有已繳付之費用不設退款。未能出席工作坊者，將當作自動放棄論，已繳交之費用不會退還。工作坊的收益將全數撥捐大華銀行「藝 • 坊」慈善機構。
I agree to complete the registration by making payment through the designated method within 2 days of receiving the payment email issued by UOB Art Academy. I also understand that once registration is successful, it cannot be transferred to another workshop or cancelled. Please be advised that all fees paid are non-refundable. Participants who do not attend will forfeit their spot without a refund. All proceeds go to the UOB Art Academy Limited, a registered charity organisation in Hong Kong.

本人同意大華銀行「藝 • 坊」將拍攝及或攝錄本人及／或子女的圖像、聲音、錄像用於市場推廣、教育、展覽或研究用途。本人授權大華銀行「藝 • 坊」編輯、使用和重用所述作品，並將之用於非牟利或大華銀行及大華銀行「藝 • 坊」認為合適的任何其他專業用途的權利，包括但不限於：印刷刊物、公共放映、網上短片及所有其他形式的媒體。本人明白所得之圖像和攝錄歸大華銀行「藝 • 坊」所擁有，本人不會收取有關酬勞或任何其他補償。本人亦免除大華銀行「藝 • 坊」及其代理人和員工因發布所拍攝或使用圖像及或錄像而可能產生的所有責任。
I permit UOB Art Academy to record photographic images and/or audio or video of me and/or my child for promotional, educational, exhibition or research purposes. I also grant to the right to edit, use and reuse said products for nonprofit purposes or in any other professional manner that UOB Art Academy believes is proper, including, but not limited to: print publications, public screening, video streaming on internet, and all other forms of media. I understand that the pictures and recordings belong to UOB Art Academy, and I will not receive payment or any other compensation in connection with the pictures and recordings. I also hereby release UOB Art Academy and its agents and employees from any and all liability that may or could arise from taking or use of the pictures and/or video.
